Respecting Your Gender Identity & Pronouns

We will discuss with you how you would like your gender identity and pronouns reflected in your chart to ensure your identity is honored in all interactions.


Minimizing Information Sharing

We always abide by the Health Insurance Privacy and Portability Act and keep your record completely confidential. When required by law (or by your request) to share medical information (e.g., for insurance claims or legal requests), we provide only the minimum necessary details. If any request for information arises, we will always contact you before releasing records and will seek legal counsel if clinical notes are subpoenaed.
